Congratulations on your acceptance to the School of Nursing at Monroe College! Now that you’ve been accepted, you’ll need to take a few important steps to complete the enrollment process.

NOTE: These documents must be submitted to the School of Nursing prior to the beginning of the semester. Failure to do so willdelayyou from starting the program.

Submit your health forms As a requirement for registration, the School of Nursing requires all students to submit documented proof of:

• A physical exam within the last year
• Physical Form must be filled out and signed by a physician
• Tetanus-Diphtheria booster within the last 10 years (required)
• Negative PPD/CXR Tuberculosis test or chest x-ray within the last 6 months (required)
• Two MMRs: measles, mumps, rubella vaccine and titers (required)
• Hepatitis B series and Chicken Pox Vaccine and titers (required)
• Meningitis and Hepatitis A vaccines (strongly recommended)

Order uniform As a nursing student it is mandatory that you wear a uniform in the classroom and clinical setting. Uniforms are ordered online through Meridy’s Uniform, Please review the LPN order form or RN order form for ordering instructions.

Conduct a background check Please be advised that prior to your clinical learning experiences at a health care facility, it is required that a criminal background check is completed in order to meet the facility's clinical placement policy or requirements. The background check must purchased through CertifiedBackground.com.

Contact your financial advisor As a prospective student, you are assigned a financial advisor to assist you and your family through the financial aid and payment process. Every incoming student must be cleared by financial aid before registering for classes.

Orientation As an accepted student it is mandatory that you attend the College’s new student orientation in addition to School of Nursing’s’ orientation. The dates are to be announced.
