Document Checklist for new students

The following are forms that you may be required to bring to the Financial Aid Officein order to receive a financial aid estimate. Please speak with aFinancial AidCounselor to determine which forms apply to your situation.

New Applications:

Monroe College Application Form
Social Security Card
Permanent Resident Card
Naturalization Certificate(if applicable)
Marriage Certificate(if applicable)
High School Transcript

Forms Filed at another institution:

Duplicate Student Aid Report (SAR) Request Form
TAP Change Form

Transfer Documentation:

Previous College Official Transcripts
Transfer Student Understanding

Proof of Income Documents: (One or more may be required) for all students

SIGNED Federal and State Returns for previous year and W-2
W-2 Statement & SIGNED Affidavit of Income (If tax forms were not filed)
Final Pay Stubs & SIGNED Affidavit of Income (If tax forms were not filed)
Proof of Unemployment Insurance (Form 1099-G)
Previous Year P.A. (Public Assistance) Budget Printout
SIGNED Affirmation of Non-Taxable Income
SSA or SSI Award Letters for previous and current years
Statement of Financial Support
