Requesting Services

  1. You must self-identify in order to receive accommodations and related services. You can do this by informing an admissions counselor during the admissions process, by filling out the self-identification form in the pamphlet or by contacting a disability counselor directly.
  2. Meet with a disability counselor for an initial screening. This is an opportunity for you to learn more about available services and for the counselor to learn more about you. (See below for disability counselors.)
  3. Provide the office with up-to-date documentation stating the nature of your disability (diagnosis,) how it affects you in an academic setting, and a list of possible accommodations. Examples of documentation include letters from a treating physician or documentation from your previous high school.
  4. Requests for services need to be made each semester you are enrolled, even if you have previously received services.
  5. It is also helpful to request services as soon as possible so that your weekly schedule allows for you to arrange transportation or make accommodations for disability-related needs.
