Student Health Requirements
Immunization Record
The immunizations listed below are required for entry to Monroe University. Students manage their health records (required forms) in their admissions portal.
Meningitis
New York State law (Public Health Law 2167) requires us to distribute information about meningococcal disease and vaccination to all students. If you decide not to receive the meningococcal meningitis vaccine, you must sign the meningitis waiver on the mandatory meningococcal meningitis response form.
Measles, Mumps, and Rubella (MMR)
All students enrolled in a New York State College must comply with New York State Public Health Law 2165. Failure to meet these requirements may result in the inability to attend classes and/or a registration hold.
- Measles: Two doses of live measles vaccine is required. The first dose must have been received no more than four (4) days prior to the student's first birthday and the 2nd dose no less than 28 days after the first dose. (MMR series fulfills this requirement.)
- Mumps: One dose of live mumps vaccine administered no more than four (4) days prior to the student's first birthday. (MMR series fulfills this requirement.)
- Rubella: One dose of live rubella vaccine administered no more than four (4) days prior to the student's first birthday. (MMR series fulfills this requirement.)
Acceptable forms of MMR immunization proof include:
- Childhood immunization records showing the exact dates of shots
- Positive blood test results (titres)
- The completion of the Monroe Immunization Record Verification Form by your health care provider, including signature.
International students must submit their immunization records translated into English.
All clinical students (Diagnostic Medical Sonography, Medical Assisting, Surgical Technology, Nursing) have immunization and health requirements in addition to the requirements for all Monroe University students. You will receive further information upon acceptance.
Students seeking an exemption from immunization requirements for health or religious reasons must submit a formal exemption request along with supporting documentation. Please send your written request to sdelllano@monroeu.edu for review and approval.
